Our costs are simple and transparent. There are no hidden fees, no sign-up fees, no long-term commitment, or complex invoices,
rather we simply implement a minimum membership requirement of 6 months. You'll know exactly what you're paying,
exactly what you're getting, and can opt out any time after the minimum has been met. Lighthouse Family Medicine has
established a straight-forward payment solution for our patients to receive high-quality, highly personal, healthcare.

OPTION ONE:
Personalized Primary Care Membership with Insurance
$150/mo per person 26+
$100/mo per person 0-25
Family Discount:
10% for 2 members
15% for 3 members
20% for 4+ members
Insurance is billed for office visits. Copays, coinsurance, deductible are collected when due.
OPTION TWO:
Personalized Primary Care Membership without Insurance
$275/mo per person 26+
$150/mo per person 0-25
Family Discount:

8 in-person office visits
1 set annual basic labs
1 set 6 month basic labs
OPTION THREE:
Personalized Primary Care Membership with Medicare
$150/mo per person
This Option Includes:
8 in-person office
visits per year
Labs billed through
Lab Corp.
With any of our membership options, immediate family members (parent, child, sibling) visiting from out-of-town may access services for up to two (2) weeks during a calendar year. Medical services for visitors are charged on a fee-for-service basis.
